Please wait for the page below to appear. If delayed, click "Reload" or "Refresh" on your toolbar.

Please note: thanks to recent improvements in online database retrieval, we hope that a substantially upgraded system for accessing source files will be implemented later this year. Please feel free to submit searches in the interim, but limit queries to five per week, and allow at least 60 to 90 days for processing. (Turnaround time next year should drop to ten workdays.) We appreciate your interest, and hope that you will find this resource more beneficial when completed.

The fine print: this submission form has been designed using industry standard configurations, and has been tested to perform well in the vast majority of hardware and software environments. If you have any problem sending in the form above, please check your browser software to make sure the settings are correct. Using Netscape, look under "Options" then "Mail and News Preferences" to see if your settings for Servers and Identity are OK. Using Internet Explorer, check "View" then "Internet Options." Most users already have these settings correct, and find that the form above works well without changes.

Please remember that the configuration of the computer you are using to view this page may effect how it works as much as the hardware and software we used to build it. Naturally, we can only maintain the system at this end, because we can't tell how the computer you are using to view the page is configured. You may want to contact your own technical support staff if you need help with any of the suggestions above, or if you don't fully understand the way your Web browsing software works. Fortunately, the overwhelming majority of users find that their current configuration is fine.
